Motorcowboy, a US company, offers made to measure boots.  Of special interest to Star Trek fans, various boots from different series, movies, etc.  I can recommend the site, as have ordered a pair of custom Star Trek boots of The Original Series, which I adore.  Cost alot, but worth it if a serious fan!
